Abdominoplasty, often called a “tummy tuck,” is a very popular surgery. Tummy tucks help to flatten and shape the abdomen and waist by removing excess fat and skin and tightening muscles. Who Is A Good Candidate For A Tummy Tuck? The best candidates for abdominoplasty are in good physical condition with pockets of fat or loose skin that haven’t responded well to diet and exercise. Abdominoplasty can also be especially helpful for women with stretched skin and muscles from pregnancy. Liposuction Techniques The liposuction procedure begins with the injection of a solution composed of saline (salt water), a local anesthetic called lidocaine, and a small amount of adrenaline to decrease bruising and bleeding. This is known as either the tumescent or super-wet technique, depending on the ratio of the fluid to the fatty tissue being removed. Once this solution has been introduced into the area planned for liposuction treatment, the doctor proceeds with one of two different methods of liposuction currently approved by plastic surgeons: suction-assisted lipectomy (SAL) or laser-assisted lipectomy. In both methods, a tiny incision is made in as inconspicuous a place as possible. In SAL, a cannula (small tube) is inserted and moved back and forth beneath the skin, breaking up the fat layer and suctioning it out. The laser-assisted method may help break up the fat cells prior to suction aspiration.

As we age, the skin on our hands can change just as much as the skin on our face, resulting in thinner, sagging skin and an increased visibility of the tendons and veins. Hand rejuvenation is a restorative treatment that combines skin fillers such as Restylane Lyft and Radiesse and native fat with laser treatments such as Fraxel® to replace lost volume and improve the overall appearance of the hand. The skin of the hand is first injected with a biocompatible filler that stimulates collagen production and thickens the skin, making tendons and veins less visible. Desired results can usually be achieved after a series of two to three treatments spaced six weeks apart. Laser treatments work with the filler to improve the texture of the skin while also removing sun spots and other unwanted blemishes. Saline And Silicone Gel Breast Implants Breast implants are silicone shells filled with either saline (salt water) or silicone gel. Both implant types are very safe, and each offers its own advantages. We will help you decide which kind is right for you. During surgery, we will place the implants behind each breast, underneath either breast tissue or the chest wall muscle. After many years, the U.S. Food and Drug Administration (FDA) has approved silicone breast implants for breast augmentation surgery. We are proud to offer patients the option of silicone gel-filled breast implants, since they are much softer and feel more natural than saline-filled breast implants. Breast Augmentation Surgery – Incision Techniques The breast augmentation procedure lasts one to two hours and is typically performed with general anesthesia. Incisions are made in inconspicuous places on the breast to minimize scar visibility. These may be located: In the crease on the underside of the breast (inframammary), Around the areola, the dark skin around the nipple (periareolar), or In the armpit (transaxillary). The breast is then lifted, creating a pocket into which the implant is inserted. Saline implants may also be placed endoscopically through an incision in the navel. This minimally invasive breast enhancement technique is known as a transumbilical breast augmentation or TUBA approach. Please note this technique is not ideal due to lack of control in implant placement and difficulty in subpectoral positioning. Breast Augmentation Surgery – Placement Options Implant placement beneath the muscle of the chest offers a few advantages over placement beneath the breast tissue only. These include reduced risk of capsular contracture (post-operative tightening around the implant) and less interference with mammogram examinations. Possible disadvantages include slightly increased post-operative discomfort. We will discuss which option may be best for you. After the implants have been inserted and positioned beneath the nipples, the incisions are stitched closed and bandaged. The patient is then placed in a soft support bra. Recovery Most patients feel tired and sore after breast augmentation surgery, but this usually passes in a day or two. Many patients return to work within the week. The stitches are self-dissolving and any post-operative pain, swelling, or sensitivity will diminish over the first few weeks. Scars from breast augmentation incisions will begin to fade in a few months and will continue to fade for months or years. Complications Complications following breast augmentation surgery are uncommon and usually minimal. They may include capsular contracture, swelling and pain, infection around the implant, a change in nipple sensation, milk production if you nursed a baby within a year before the procedure, and breakage or leakage of the implant (implant rupture) as a result of injury or the normal compression and movement of your breast. If a saline implant ruptures, the implant will simply deflate in a few hours and your body will absorb the salt water. A silicone implant leak may not be as easily detected and an MRI may be helpful for diagnosis. In either case you will need to visit your surgeon for an assessment.

A woman’s breasts may droop as a result of the natural effects of aging, heredity, gravity, pregnancy, breastfeeding or weight loss. A breast lift, also called a mastopexy, is performed to return youthful shape and lift to breasts that have sagged or lost volume and firmness. What Can A Breast Lift Do For Me? Breast lifts rejuvenate the breasts by trimming excess skin and tightening supporting tissues to achieve an uplifted, youthful contour. After a mastopexy, the breasts are higher on the chest and firmer to the touch. Breast lifts can also reposition and reduce the size of the areola—the dark skin surrounding the nipple—which may have stretched or drooped. Breast size does not change after a breast lift, nor does the fullness or roundness in the upper part of the breasts. Women who desire larger, smaller or more rounded breasts may want to consider a breast augmentation or breast reduction together with a breast lift. Am I A Good Candidate For A Breast Lift? The best candidates for breast lift are women whose breasts meet some or all of the following conditions: Breasts sag Breasts have lost shape or volume Breasts are flat, elongated, or pendulous (hanging) Breast skin and/or areola is stretched Nipples or areolas point downward Nipples or areolas are located in the breast crease when breasts are unsupported One breast is lower than the other It is also very important that breast lift candidates: Maintain a stable weight Are generally healthy Do not smoke Discuss realistic goals with their plastic surgeon Who Should Not Consider A Breast Lift? Women planning to have children are advised to postpone surgery, since pregnancy and nursing can counteract a breast lift’s effects by stretching the skin. Mastopexy does not affect breast size or upper fullness. Women who desire larger, smaller or more rounded breasts may want to consider a breast augmentation or breast reduction either together with a breast lift or instead of a breast lift. The Technique Dr. Ganchi Chooses… Mastopexy may be performed in a hospital, an outpatient surgery center or a surgeon’s office-based facility. It is usually done on an outpatient basis under general anesthesia, and lasts from 1 ½ to 3 ½ hours. There are several types of incision methods that can be used when performing a breast lift. The technique a surgeon chooses depends on the patient’s: Breast size and shape Degree of sagging Size and position of the areolas/nipples Amount of excess skin Skin quality And other factors. The three most common incision types are two rings around the areola in a doughnut shape (small-incision mastopexy, generally only recommended for patients with small breasts and minimal sagging); around the areola and down to the breast crease in a lollipop shape; and the lollipop with an additional half-moon incision along the breast crease, in an anchor shape. In all cases, breast lift surgery begins with administration of anesthesia or IV sedation. Dr. Ganchi makes the necessary incisions, and then lifts and reshapes the breast tissue into its new, rejuvenated contour. She moves the nipple and areola higher on the breast and removes extra skin around the perimeter if the areola is enlarged. Finally, Dr. Ganchi trims the excess breast skin that resulted from poor elasticity. Stitches layered deep throughout the breast tissue support the lifted breasts. When the mastopexy is complete, the skin is closed with stitches, tissue adhesive and/or surgical tape. Some of the incisions are hidden in the breast crease. Others will be visible. All scars will mature with time. What Is The Recovery From A Breast Lift Like? After surgery, the breasts are wrapped with gauze dressings, and you may need to wear an elastic bandage or a surgical bra. After a few days, this is replaced with a soft support bra, which is worn 24 hours a day for about a month. The breasts will probably be bruised, swollen, and uncomfortable after surgery, but this will pass in a few days. Any numbness in the breasts and nipples should lessen as swelling subsides. Patients will wear a soft support bra and return to most daily activities within 5-7 days. What Are The Risks Of Mastopexy? Possible complications of a breast lift include: Bleeding Infection Temporary or permanent numbness in the nipple or areola Uneven positioning or shape of breasts or nipples Widening of scars Poor incision healing Fluid buildup Dr. Ganchi will discuss all the risks and benefits of a breast lift with you. You will be able to see the results of your mastopexy immediately after surgery, and you may become even more satisfied as swelling goes down and incision lines fade.

Many women are born with breasts that are uneven in size and shape, which may or may not be noticeable to others. These differences in breasts are often undetectable to patients themselves, causing no problems at all, while other women can have obvious asymmetry, which can cause both an unbalanced appearance and emotional distress. Asymmetric breasts can be corrected through surgery that either enlarges the smaller breast or reduces the larger one. Implants can be placed in smaller breasts or a breast reduction can be performed on larger breasts. If the nipples appear at different heights, surgery can relocate one or both of the nipples. All of these options produce breasts that are more similar in appearance, working with each woman’s unique body to achieve the best possible results. Creating a more balanced appearance can help improve physical balance, self-confidence and body image.

We specialize in revision breast augmentation for patients who are not satisfied with their results after undergoing breast augmentation at other practices. These patients’ unhappiness may arise from the implant shape, size, or placement or because of a post-surgical complication such as wrinkling, implant displacement, capsular contracture (hardening around the implant) or symmastia (implants drift together). Reasons For Revision Breast Augmentation After their first breast augmentation, many women discover that they actually want their breasts to be larger or smaller than the results achieved during the initial procedure. Different Implant Size This is the most common reason that patients seek revision surgery. During this type of revision procedure, the same incision is used to remove the old implants and insert a different implant based on the patient’s individual body. Small adjustments may be required in order to accommodate the new breast size. Patients seeking changes to their implant size are advised to wait at least three months after the initial procedure before undergoing revision augmentation. After three months, swelling has subsided and the implants have settled, allowing patients to evaluate the true size of their implants. From there, revisions can be made if desired. Implant Leaks/Ruptures Saline breast implants that begin to leak are often noticeable, as the leaking breast will gradually decrease in size. Patients with silicone implant leaks may notice small changes in the shape of the breast, but should have any suspected leaks or ruptures confirmed by a plastic surgeon. Your plastic surgeon may perform an evaluation through a mammogram or MRI to evaluate the status of your implants. A leaking or ruptured implant should be replaced right away. This type of revision can be done through the same incision as the initial procedure. The procedure is often easier and less painful than the first procedure, and patients can benefit from a faster recovery as well. Implant Complications There is always a risk of certain complications and side effects from a breast augmentation procedure. Some of these complications may include wrinkling and rippling of the implant, capsular contracture and shifting of the implants. Wrinkling and rippling occur most often in women with thin skin or a small amount of natural breast tissue. These ripples can often be felt when touching the breast and may even be visible under the skin. Capsular contracture involves hardening of the breasts and a change in shape as well as scar tissue developing around the implant. There are several solutions available to help correct these complications. Changing the size, shape, position or type of implant is often helpful in relieving these problems and correcting the look and feel of the breasts.

Nipple Eversion And Reconstruction Inverted nipples, which point inward rather than out from the breasts, can be a source of self-consciousness as well as concern about the ability to breastfeed. However, this condition may be corrected through a nipple eversion procedure. This surgery involves releasing or stretching internal connective tissue that has been pulling the nipple inward. The eversion procedure is typically performed with the use of a local anesthetic. An incision is made in the areola to access the connective fibers. The nipple is then placed in an outward position and held there using sutures. The sutures dissolve as the breast tissue heals and the nipples remain pointing outward. Nipple eversion surgery can preserve nearby milk ducts to maintain a patient’s ability to breastfeed in the future. Nipple and areola reconstruction is typically performed to create nipples if they are missing due to mastectomy, injury or a congenital defect. It is often the final step in breast reconstruction surgery, where the nipple is formed after the breast mound has settled into place so that the new nipple matches the appearance and position of the natural one as closely as possible. Nipple reconstruction may be performed using one of several techniques, including tissue and cartilage flaps, skin grafts and tattooing. Nipple Reduction Many men and women are embarrassed or bothered by the larger size of their nipples. Patients may be born with larger nipples, or they may develop after breastfeeding. Whatever the cause may be, larger nipples can be reduced through a simple surgical procedure that offers immediate results and minimal risks, eliminating patients’ worries about nipple size and shape. Nipple reduction is commonly performed during a breast reduction procedure, so that the size of the nipple will match the new size and shape of the breast. Larger nipples often create a matronly or older appearance, although it commonly occurs after breastfeeding or in naturally large breasts. A reduced nipple size can help create an overall youthful appearance to the breast, and helps maximize the aesthetic results of a breast reduction or breast lift procedure. It is important for patients to realize that the nipple and the areola are not the same thing. The nipple is the projected part of the breast, while the areola is the darker pigmented skin that surrounds the nipple. Many patients undergoing nipple reduction may also seek areola reduction in order to achieve their desired results.

Breast reduction surgery, also known as reduction mammaplasty, is usually done to provide relief from these symptoms. Performed under general anesthesia, this procedure removes fat and glandular tissue and tightens skin to produce smaller, lighter breasts that are in a healthier proportion to the rest of the body. Benefits Of Breast Reduction Women with large, heavy breasts may suffer physical pain from the weight of their breasts and may experience social discomfort due to unwanted attention to their breast size. Because of this, many women turn to breast reduction as a remedy. Large, heavy breasts can also cause strain on a woman’s body. The extra weight of heavy breasts can be harmful to posture and may lead to spinal problems. In addition to causing back, neck, and shoulder pain, excessively heavy breasts may also lead to breathing problems and other serious health issues. Breast reduction surgery can help a woman correct her posture and remove strain on the back, neck, and shoulders, reducing overall pain. In addition to seeking relief from the pain caused by heavy breasts, many women seek breast reduction surgery because they are uncomfortable with the unwanted attention that their breast size draws. This unwanted attention may cause a woman to become uncomfortable with her appearance. Breast reduction surgery can help a woman regain her self-confidence and positive self-image, as well as provide the motivation to participate in a wide range of physical and social activities. Incision Techniques Breast reduction surgery generally takes two to four hours, depending on the patient’s specific case, and is performed under general anesthesia. The size and shape of your breasts, as well as the desired amount of reduction, helps determine which type of incision is right for you. The different types of incisions for a breast reduction include: Vertical Incision Breast Reduction Surgery – The vertical incision technique, also known as the short scar or LeJour breast reduction, typically results in shapelier breasts and less scarring than traditional breast reduction surgery. This is Dr. Ganchi’s preferred technique, as it is often the best way to reduce larger breasts while minimizing scarring, resulting in satisfying patient outcomes. Vertical incision breast reductions are a newer, popular alternative to the traditional method, especially for women who do not require extensive tissue removal. Incisions in this technique extend around the areola and vertically down to the breast crease, making a lollipop pattern. Vertical incision breast reductions eliminate the need for the horizontal incision along the breast crease that is used in traditional breast reduction surgery, resulting in much less scarring. Women with moderately large breasts are the ideal candidates for vertical incision breast reductions. Vertical incision breast reduction surgery may not be a good choice for women requiring the removal of excessive amounts of fat and breast tissue. Traditional Breast Reduction Surgery – The traditional approach to breast reduction surgery is a very effective method for alleviating discomfort caused by large, heavy breasts. The traditional method, also called an anchor pattern or Wise pattern reduction, may be beneficial for women requiring a large amount of tissue removal. In the traditional breast reduction surgery technique, incisions are made horizontally under the breast in the crease and extend vertically up to and around the nipple, creating an anchor pattern. Because traditional breast reduction surgery allows for excess skin removal both vertically and horizontally, it may be recommended for women with extremely large breasts. The anchor shaped incision also allows surgeons to remove more tissue than other methods, maximizing breast reduction results. Although the anchor pattern method can be useful in reducing excessively large breasts, it leads to more scarring than other breast reduction surgery techniques. These scars can often be hidden by bathing suits and even low cut tops.

Gynecomastia is a common condition that affects nearly half of all men, resulting in localized fat and/or glandular tissue in the breasts. Male breast reduction may be performed using liposuction, surgery or a combination of the two, depending on the amount and type of tissue found in the breasts. If the breasts consist of mostly fatty tissue, liposuction may be used to suction out fat from an incision in the nipple or underarm areas. For breasts with an excessive amount of glandular tissue, excision surgery may be performed, which requires cutting away the excess fat, skin and tissue through a larger incision. Your surgeon will determine which technique is best for you based on your individual goals for surgery. For most patients, breast reduction surgery produces a flatter, more well-defined chest that can last for many years, as long as patients maintain a stable weight.
Eyelid surgery, also called an eyelid lift or blepharoplasty, can rejuvenate puffy, sagging or tired-looking eyes by removing excess fat, skin and muscle from the upper and/or lower eyelids. Eyelid surgery may be performed for cosmetic reasons or to improve sight by lifting droopy eyelids out of the patient’s field of vision. What Can Eyelid Surgery Do For Me? How Is A Blepharoplasty Performed? Eyelid surgery is usually performed in an office, usually with local anesthesia and IV sedation. The procedure can take less than 45 minutes. Incisions are made along the eyelids in inconspicuous places: in the natural creases of the upper lids for upper eyelid blepharoplasty and just below the lash line on the lower lids for lower eyelid blepharoplasty. If no skin needs to be removed, Dr. Ganchi will likely perform a transconjunctival blepharoplasty, where the incision is made inside the lower eyelid and there are no visible scars. In all cases, Dr. Ganchi then removes excess skin from the eyelids using a scalpel, laser or radiofrequency (RF) tool. Fat may be removed as well, before the incisions are closed with fine sutures, tissue glue or surgical tape. What Is Recovery From Eyelid Surgery Like? Bruising, swelling and a sense of tightness in the eyelids are common after eyelid surgery. These should subside within 2-4 weeks. Most patients resume activities in 3-5 days, when non-absorbable stitches are removed, and return to work in about 10 days. By one to two months after surgery, you should see the true results of your eyelid lift: a more awake and cheerful look around the eyes, and greater self-confidence. Contact lenses may not be worn for two weeks. Eyes can be sensitive to light and wind and may be slightly irritated for a short while; your surgeon may prescribe eye drops and cool compresses to ease any discomfort.

The neck may sag as a result of aging or weight loss, and is often affected before the face. Loose neck muscles can lead to an appearance of bands on the neck or drastic drooping of the skin. A neck lift is a surgical procedure that can smooth and tighten the skin on the neck and provide a more toned and youthful appearance. The results of this procedure offer a drastic improvement to the appearance of the neck and subtle changes to overall appearance. A neck lift may be performed alongside a facelift, or may be performed alone, after which your friends and family may not even realize that a surgical procedure was performed. A neck lift procedure includes both a cervicoplasty to remove excess skin and a platysmaplasty to remove or alter muscles in the neck. Liposuction can also be used to remove excess fat during a neck lift. This takes two to three hours and may be performed under general anesthesia or local anesthesia with sedation, depending on the preference of the patient. Most procedures are performed within an outpatient basis. Swelling and bruising may occur shortly after the procedure and last up to ten days. Most people can return to work and other regular activities within two weeks after a neck lift procedure. There are certain risks involved in a neck lift, as there are with any surgical procedure, including problems with anesthesia or infection. There is also a risk of facial nerve damage that may occur during surgery and can result in muscle paralysis. Browlifts can revitalize drooping or lined foreheads, helping you to look less angry, sad or tired. Using either traditional or minimally invasive (endoscopic) methods, browlifts involve the removal or alteration of the muscles above the eyes to smooth lines and raise the eyebrows for a more youthful appearance. The procedure is often combined with other operations such as blepharoplasty (eyelid surgery) and facelifts to improve the look of the eyes and other areas of the face. While browlifts are typically performed on patients aged 40-60 to counteract the effects of aging, anyone with lined or inelastic skin on the forehead – whether it’s caused by muscle activity or inherited conditions – is eligible. The procedure is usually performed in an office-based facility or surgery center under local anesthesia and sedation. Traditional browlift: First, the patient’s hair is tied with rubber bands near the incision site. Then Dr. Ganchi makes a coronal (headphone-shaped) incision behind the hairline, stretching between the ears and across the top of the forehead. The incision may be placed further back or made along the skull bone joints to minimize visibility in patients with thinning hair or who are bald. Then the forehead skin is lifted, tissue is removed, muscles are adjusted and the eyebrows may be lifted. Excess skin is trimmed and the incision is closed with stitches or clips. The site is washed and the rubber bands are removed. Some surgeons will protect the stitches with gauze or bandages. Endoscopic browlift: After the hair is tied as with the traditional procedure, Dr. Ganchi makes three to five short incisions (less than an inch long) behind the hairline. The endoscope is placed in one so Dr. Ganchi can see beneath the skin without having to make a large incision, while he or she lifts the skin and adjusts muscles through the other incisions. If the eyebrows are lifted they will be stitched or screwed (temporarily) into place. The site is washed and the rubber bands are removed. Some surgeons will protect the stitches with gauze or bandages.

Over time, a pierced earlobe can become stretched if a person frequently wears large or heavy earrings. In addition to creating a long or wide opening, earrings may tear completely through the lobe. A torn earlobe may also result from trauma, such as having an earring pulled off or caught on clothing. Torn earlobes can be repaired with a quick in-office procedure performed under local anesthesia. Torn earlobe repair first involves “freshening” the edges of the torn lobe by removing a small amount of skin. Then, using fine sutures, the earlobe is meticulously repaired to reconstruct a normal, rounded earlobe that matches your natural ear shape. You will be able to return home immediately after the procedure, with only a small bandage covering the stitches. The stitches will be removed after one to two weeks. Chemical Peel Chemical peels remove damaged outer layers of skin on the face to smooth texture, reduce scarring, and remove blemishes to produce healthy, glowing skin. There are three types of chemical peels, ranging from mild to strong – alphahydroxy acids (AHAs), trichloroacetic acid (TCA) and phenol – and formula strengths are tailored to each patient. Peels can be combined with other procedures such as facelifts for a younger look. Microdermabrasion Microdermabrasion improves mild to moderate sun damage, scarring, wrinkles and other superficial skin problems. A gentle stream of tiny particles exfoliates the outer layer of skin and provides a fresh, glowing, younger appearance. Only a small amount of skin is removed, so sensitive areas such as the face, chest, neck, arms and hands can be treated. Results are comparable to a light chemical peel and should be immediately visible. There is no anesthetic involved with this procedure and very few side effects; slight redness, soreness, flaking and scabbing of the skin can occur. The actual pulse of the laser feels like a rubber-band snap across the skin but is easily tolerable by most patients. There is no recovery time associated with this procedure and it can be repeated until proper results have been achieved, normally with a month or two between sessions to determine the full effectiveness of an individual treatment. Tattoo Removal Many people have second thoughts about their tattoos. Fortunately, you don’t have to live with your decision forever. Highly precise lasers can lighten or remove your tattoo by focusing or pulsing light on the ink particles, which then convert the energy to heat and break up into tiny pieces that are absorbed harmlessly by the body. All this can be accomplished without damaging the surrounding tissue. Results can generally be seen within a few weeks, although larger and darker tattoos may require more treatments than smaller, lighter ones. Occasionally treated skin will look whiter (paler) than the skin around it, but this is usually temporary and fades in the months following treatment. Fraxel Laser Fraxel® laser systems use fractional-resurfacing technology, in which only a fraction of an area of skin receives laser light, to significantly improve skin texture and and remove pigmentation.
